Authorization and Consent For Minor -- (otherwise known as a "permission slip.")

VCF Event Worksheet -- If you're planning an event that involves VCF, this is the form you'll need to fill out. Please fill it out legibly and completely, then read and sign the back of the form. Every line must be addressed and filled in. If a particular line is not applicable to you, then enter "N/A" in the space provided. If your event is approved, a letter of confimation will be sent to you.

VCF Worship Team Questionnaire -- (fill out and send to the church, or drop off at the church office.)
